HCA Healthcare medical bill watch-list.
Largest for-profit hospital chain in the US. ~180 hospitals across 20 states.
Common billing patterns we catch
- Separate claims from hospital, ER physician group, and radiology — verify each is in-network
- NCCI bundling errors when multiple line items overlap (e.g. wound care + sutures)
- Modifier 25 or 59 added inappropriately to evaluation visits
Context on HCA Healthcare billing
HCA bills are some of the most-audited by Audra users — partly volume (HCA serves a huge slice of the country) and partly because their physician-staffing model includes a high share of contract emergency-room physicians. That mix produces a fair number of NCCI bundling errors and modifier-misuse cases when a single ER visit results in claims from the hospital, the ER physician group, and an external radiology read. Out-of-network surprise bills used to be rampant pre-No-Surprises-Act; some predate that protection.
